CERTIFICATE OF APPEARANCE

Requirements:

1. Personal appearance of the applicant
2. Original and two (2) photocopies of the data page (with signature) of valid passport
3. One passport size photo of applicant
4. Fee of 22.50 euros in cash (plus 9 euros for expedite processing)

CERTIFICAT DE COUTUME AU MARIAGE
Requirements:

1. Photocopy of the data page (with signature) of the valid passport of the Philippine citizen
2. One passport size photo of the applicant
3. Original and photocopy of Certificate of No Marriage (CENOMAR) issued by PSA and apostilled by DFA
4. If widow/er, original and photocopy of Marriage Certificate or Report of Marriage and Death Certificate issued by PSA
5. If annulled or divorced, original and photocopy of Marriage Certificate or Report of Marriage with annotation issued by PSA
6. Fee of 22.50 euros in cash (plus 9 euros for expedite processing)

CERTIFICAT DE COUTUME AU PACS

Requirements:

1. Photocopy of the data page (with signature) of the valid passport of the Philippine citizen
2. One passport size photo of the applicant
3. Original and photocopy of Certificate of No Marriage (CENOMAR) issued by PSA and apostilled by DFA

4. Original and photocopy of document to establish filiation with parent/child, issued by PSA
5. Fee of 22.50 euros in cash (plus 9 euros for expedite processing)

CERTIFICAT DE COUTUME A L’HEREDITE

Requirements:

1. Photocopy of the data page (with signature) of the valid passport of applicant
2. One passport size photo of applicant
3. Original and photocopy of Certificate of Birth and/or Marriage of applicant issued by PSA
4. Original and photocopy of the Death Certificate of the Deceased
5. Fee of 22.50 euros in cash (plus 9 euros for expedite processing)

CERTIFICAT DE COUTUME A LA LEGITIMATION

Requirements:

1. Photocopy of the data page (with signature) of the valid passport of applicant
2. One passport size photo of applicant
3. Original and photocopy of Certificate of Birth and/or Marriage of applicant issued by PSA
4. Original and photocopy of document to establish filiation with parent/child, issued by PSA
5. Fee of 22.50 euros in cash

CERTIFICAT DE COUTUME A L’ADOPTION

Requirements:

1. Photocopy of the data page (with signature) of the valid passport of applicant
2. One passport size photo of applicant
3. Original and photocopy of Certificate of Birth and/or Marriage of applicant issued by PSA
4. Original and photocopy of document to establish filiation with parent/child, issued by PSA
5. Fee of 22.50 euros in cash (plus 9 euros for expedite processing)

FOR MAILED-IN APPLICATIONS FOR CERTIFICAT DU COUTUME:

• Download the “Service Request Form” from the Embassy website -http://parispe.dfa.gov.ph/downloadable-forms;
• Fill out the form and send it by mail to the Embassy together with the required documents listed under the Certificat du Coutume requested;
• There is no need to send the original passport.
• Enclose a self-addressed registered return envelope (recommandé avec avis de réception). Please write the correct mailing address and record your tracking number. The Embassy does not accept payment for stamps and mailing services;
• The fee of 22.50 euros shall be in chèque de banque payable to Ambassade des Philippines.  Personal checks are not accepted. Do not mail payment in cash.
